3.	Do you have at least 3 years of paid childcare experience in the last 7 years?	YES NO
4.	Can you provide at least 2 childcare references who are not family or friends?	YES NO
5.	Are you willing to accept all types of compensation, both same-day compensation and a paycheck with taxes taken out?	YES NO
6.	Is your background free of any felony or misdemeanor convictions or any currently pending charges?	YES NO
7.	Are you 21 or older?	YES NO
